How to request an ESA evaluation from Miami

1. Complete the free pre-screen. Share basic information so the team can explain next steps and current pricing.
2. Meet with a Florida-licensed clinician. The clinician conducts an individualized evaluation and makes the clinical decision independently.
3. Receive documentation only if appropriate. If supported by the evaluation, the clinician may provide housing documentation. The housing provider reviews an accommodation request separately.

Residents of Miami and Miami-Dade County can request the statewide telehealth service online. A letter does not guarantee a particular housing outcome.

Keep the request and supporting documents in a private, traceable channel. If the provider asks for a form, ask how to submit it and keep a copy; Florida law does not allow a housing provider to deny a request solely because a person did not use its routine form. The provider may still review the facts and reliable supporting information allowed by law.

Florida Statute § 760.27 allows a housing provider to offer a routine process, but says it may not require a specific form or notarized statement or deny a request solely because the person did not follow that routine method. Individual circumstances and other applicable rules still matter.
