How to request an ESA evaluation from Port St. Lucie

2. Meet with a Florida-licensed clinician. The clinician conducts an individualized evaluation and makes the clinical decision independently.
3. Receive documentation only if appropriate. If supported by the evaluation, the clinician may provide housing documentation. The housing provider reviews an accommodation request separately.

Tell the housing provider for your current home about the accommodation you are requesting and ask what supporting information it needs. A move changes the dwelling and may change the reviewer, so do not assume an earlier submission automatically carries over.
